Sausage Hand by Andrew Smith
The story follows a mentally unstable character named Sausagehand in his exploration of a mall and its inhabitants. He meets a girl named Ami but is turned against her by his dark alter-ego Pygmalion. In the end he discovers the truth about Ami and proceeds to fully withdraw into the dark recesses of his own mind.
